If you use any other form of work reference than the sample, then the following information
MUST be included:

– Full employer legal name, address and contact details
– Your full name as shown on passport
– Work period (start- and end-date [month/year to month/year] or a clearly indicated open end date to show that the work is still ongoing)

– Job description
– Date and place of signing the reference
– Name of person signing the reference
